Clinical document · revised July 2026
The Protocol
Five steps, twelve markers, three programmes. Everything below is what a prescribing clinician actually does with your file — and what it costs before you ever speak to us.
How it runs
- 01Intake
You tell us what changed
A twenty-minute written intake and, if you want one, a call. Symptoms, history, medications, and what you actually want back — sleep, drive, the gym, the afternoon.
Day 0 - 02Panel
We draw twelve markers
A fasted morning draw at a lab near you, or a mobile phlebotomist at your door. Results land in your portal, annotated, not dumped.
Day 2–5 - 03Plan
A physician reads them with you
Thirty minutes with a prescribing clinician. Dose, ester, route, ancillaries, and the specific numbers we will watch. You leave with the plan in writing.
Day 7 - 04Titration
We adjust to the number, not the story
Medication ships to your door. Week six brings a short panel; the dose moves if the panel says so. Most men change dose once.
Week 6 - 05Review
Twelve weeks, then quarterly
A full panel at week twelve, then every quarter. Haematocrit and PSA are non-negotiable checkpoints — we will pause a protocol over either.
Week 12
The twelve markers
Drawn fasted, before 10 a.m., before a single dose. Each one earns its place — we do not run panels for the invoice.
| Marker | Why it is on the panel |
|---|---|
| Total testosterone | The headline number, and the least interesting one on its own. |
| Free testosterone | What is actually available to your tissue. The number that usually explains the symptoms. |
| Estradiol, sensitive | Too low is as bad as too high — joints, mood, libido. |
| SHBG | Decides how much of your total is free. High SHBG changes the whole plan. |
| Luteinising hormone | Tells us whether the signal or the factory is the problem. |
| Follicle-stimulating hormone | Reads the same axis, and flags fertility questions early. |
| Haematocrit | The most common reason we pause a protocol. Watched every panel. |
| Haemoglobin | Paired with haematocrit to separate dehydration from real thickening. |
| PSA, total | Baseline before any dose, then tracked. Non-negotiable over 40. |
| TSH | Thyroid mimics low testosterone almost exactly. We rule it out first. |
| HbA1c | Three-month glucose average. Metabolic health moves with hormones. |
| Lipid panel | Oral and high-dose protocols move lipids. We watch, and we adjust. |

Prices are the price. Lab fees, medication, shipping and clinician time are inside the monthly figure — there is no separate dispensing charge.

Who we decline
Roughly one in six men who complete an intake do not start a protocol with us. That is the protocol working, not the funnel leaking.
- Haematocrit above 52%Donate or treat the cause first. We will re-test in eight weeks.
- Untreated sleep apnoeaTreating the apnoea alone lifts testosterone in a third of men.
- Fertility plans inside 18 monthsExogenous testosterone suppresses sperm production. Different protocol.
- Active prostate or breast cancerAn absolute contraindication. No exceptions.
- Normal free T with a full symptom pictureThe answer is somewhere else. We will tell you where to look.
Questions
The five we are asked most, answered the way a clinician would answer them on the phone.
